• Name: Generic name of template

• Description: Describe the template

• Site: Select site the template belongs to

• AES Server Group: AES Server group configured at the Site/Server level

• Contact Group: Contact group configured at site/Resources level

• Discard Phone List: Enter one or more phone number you would like to prevent from being added to the user’s My History tab in CT Client application. The phone number must be unformatted
